Using NLR, PLR, and ALT*WBC Index (AWI) to differentiate between epididymo-orchitis and testicular torsion in children: A comparative study

Abstract

Background: Testicular torsion is a severe urological emergency that disrupts blood supply to the testis, leading to ischemic injury or necrosis if untreated. Early surgical intervention is critical.
Objective: The aim of this research is to contrast the diagnostic values of neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and the ALT*WBC index (AWI) - all of which can be acquired through standard diagnostic practices - to distinguish between TT and EO.
Methods: This research incorporated 75 subjects, divided into 37 patients with Epididymo-Orchitis (EO, group 1) and 38 patients with testicular Torsion (TT, group 2).
Results: The mean white blood cell (WBC) and neutrophil (NEU) counts were significantly higher in Group 2(p<0.05). Alanine aminotransferase (ALT) levels were found to be significantly higher in Group 2(p<0.05). When comparing the mean NLR, PLR, and AWI between the groups, Group 2 showed significantly higher values for NLR and AWI(p<0.05), while no significant difference was found in the platelet-to-lymphocyte ratio (PLR) (p>0.05). NLR, PLR, and AWI were assessed as potential diagnostic measures AWI showed the highest accuracy in distinguishing between the groups.
Conclusion: This study suggests that NLR and AWI could be potential markers for distinguishing between EO and TT in children, with AWI showing higher accuracy
